Requests for prescriptions are handled only during regular office hours. Please call your pharmacy when you need a prescription refill and have them fax a refill request to us at (602) 870-4512. They will contact our office for authorization. Please allow at least 72 hours to complete your refill.

If you need a new prescription, please call (602) 997-7331 to make an appointment with one of our doctors.
